Mandibular reconstruction with fibula free flap remains one of the most important techniques for restoring form and function following resection. A newly published study in Oral Oncology analyzed 75 patients and 202 osteotomy sites to better understand which factors most influence long-term osseous union.


Interestingly, researchers found that initial osteotomy gap width did not significantly predict long-term healing. Instead, stronger predictors included vascular health, osteotomy location, fibula morphology, and whether healing occurred between fibula segments or between fibula and native mandible.


Fibula-to-fibula interfaces demonstrated stronger healing outcomes, while symphyseal regions outperformed body and angle regions. The study also found that vascular comorbidities and alcohol use were associated with poorer union outcomes.
